Breast Augmentation: The Procedure

Breast augmentation is typically performed under general anesthesia as an outpatient procedure, meaning you go home the same day.

Dr. Khansa makes a small incision, most commonly in the inframammary fold (the crease beneath the breast), to minimize visible scarring. The implant is carefully positioned and the incision is closed with precision sutures.

The procedure generally takes 1 to 2 hours.

🔍 What type of breast implant should I choose?

There are two types of breast implants: silicone implants, and saline implants. Silicone implants are made up of a silicone shell filled with cohesive silicone. Saline implants are made up of a silicone shell filled with sterile saltwater solution (saline solution).

🔍 How do I choose the right implant size?

The choice of size is based on your anatomy, lifestyle, and aesthetic preferences. Depending on your body type, there is a range of breast implant sizes that will allow you to achieve a natural result, and Dr. Khansa will guide you in making your choice. Dr. Khansa will also have you try on implant sizers during your consultation to visualize potential results and help you make the right decision.

🔍 How do I choose the right implant shape?

Breast implants may be either round or shaped. Shaped implants are also known as "teardrop" or "gummy bear" implants. Most patients choose round implants, as they feel soft, and usually result in a very natural result. Shaped implants may help achieve more fullness at the top of the breast, but they can feel firmer than round implants.

🔍 Where is the incision for a breast augmentation?

The two most common incision locations for breast augmentation are inframammary (under the breast fold) and periareolar (around the nipple). Both incisions usually heal well. The periareolar incision is great when a breast lift is being done at the same time as the augmentation. Dr. Khansa will discuss the best approach for your specific anatomy and desires.

🔍 What plane is the breast implant placed in?

The "plane" that the implant is placed in refers to what tissue layer the implant pocket is created in. There are usually 4 options for planes:

Subglandular (under the breast tissue)
Subfascial (under the fascia or covering of the pectoralis muscle)
Submuscular (under the pectoralis muscle)
Dual-plane (partially under the pectoralis muscle, and partially under the breast tissue)

For the majority of patients, Dr. Khansa's preferred method is the dual-plane. This plane is ideal, because it provides a smooth, natural transition at the top of the breast, while creating a nice natural contour at the bottom of the breast.

🔍 Can a breast augmentation be performed at the same time as a breast lift?

In most cases, yes. Dr. Khansa will be able to tell you whether he thinks you need a lift with your augmentation, and whether these two procedures should be performed at the same time, or separately. The decision depends on several factors, including quality and thickness of your tissues, amount of ptosis ("sag") that you may have, and other factors.

🔍 What is capsular contracture?

Any time a breast implant is inserted into the body, the body forms a wall around it. This is called the capsule. Most of the time, the capsule is soft and healthy, and you will not feel it. In a small percentages of cases, the capsule may become thick and scarred. This is called "capsular contracture". In mild cases, capsular contracture may feel like the breast implant pocket has become harder, but it does not look different, and does not hurt. In severe cases, capsular contracture may become visible and painful. Studies estimate the risk of capsular contracture to range between 2.3% and 10.8%

🔍 Do breast implants affect breastfeeding?

According to a 1999 report by the Institute of Medicine, the milk produced by women who have breast implants does not contain more silicone than the milk of women without breast implants. In fact, babies are exposed to much higher concentrations of silicone in cow milk, formula and pacifiers. Some studies have found that women with breast implants have difficulty producing enough milk to exclusively breastfeed.

🔍 Do breast implants interfere with mammograms?

Yes, in some situations, breast implants can interfere with mammograms. Implants placed under the pectoralis muscle (submuscular and dual-plane) are less likely to interfere with mammograms than implants placed under the breast tissue (subglandular).

🔍 Does breast augmentation cause breast cancer?

There is no evidence that breast augmentation causes or increases the risk of breast cancer. A 2018 study found that women with a history of breast augmentation who then developed breast cancer were more likely to detect their cancer by palpation, and less likely to detect it by mammogram. While breast implants are not associated with an increased risk of breast cancer, they have been associated with two other types of rare cancer that can occur in the capsule around the implant, called Anaplastic Large Cell Lymphoma (ALCL) and Squamous Cell Carcinoma (CSS) (see below).

🔍 What is Anaplastic Large Cell Lymphoma (ALCL)?

Anaplastic Large Cell Lymphoma (ALCL)is a rare type of cancer that can occur in the capsule around a breast implant. ALCL is mostly associated with textured breast implants. This was first reported in 1997. As of 2024, there had been around 1,380 cases of breast implant-associated ALCL reported worldwide. The lifetime risks of ALCL for women with textured breast implants has been estimated to be around 1 in 30,000. ALCL is suspected if a woman with breast implants develops swelling or a seroma (fluid around the implant) several years after the implants are placed. In order to diagnose ALCL, the fluid around the implant is sampled, and is tested for a protein called "CD30". In many cases of ALCL, removing the implant and the capsule around it is sufficient, but in some cases, further treatment is necessary.

Squamous Cell Carcinoma (SCC) is a type of cancer that has been reported in the capsule around breast implants. It is extremely rare, with about 19 total cases reported

🔍 What is Breast Implant Illness (BII)?

Breast Implant Illness (BII) is a condition that has been reported in some women with breast implants. It includes symptoms such as decreased energy, fatigue, mental fog, and join pain. There is contradictory evidence as to whether breast implants are associated with autoimmune disease or connective tissue diseases, with some studies showing a connection, and others not showing a connection. In most women with symptoms of BII, removing the breast implants an eliminate the symptoms of BII.

🔍 Are there other options for breast augmentation if I do not want implants?

Yes! Breast augmentation using your own fat is a great option if you do not want breast implants. This technique is called fat grafting. It involves liposuction from your abdomen and flanks, and injection of the fat into your breasts. This technique requires an expert plastic surgeon like Dr. Khansa, who knows how to process the fat, where to inject it, and how much to inject. More than one round of fat grafting may be necessary to achieve your desired size.

🔍 How do I decrease my risk of complications after breast augmentation?

There are essential steps you must take to ensure a smooth recovery after breast augmentation:

Prevent infection and wound healing problems: Do not smoke, or use any form of tobacco, nicotine or vaping for 6 weeks before surgery, until 6 weeks after surgery
Accelerate your recovery: Eat a high protein diet. Drink plenty of fluids
Prevent blood clots: If you are on an oral contraceptive pill that contains estrogen, stop it 6 weeks before surgery and do not restart it until 6 weeks after surgery. You must walk at least 5 times a day, starting on the evening of surgery
Prevent pneumonia: Take deep breaths throughout the day, and cough if you need to
Prescriptions: Take prescription pain medications as instructed. Do not take herbal or homeopathic medications unless approved by Dr. Khansa, as these can cause bleeding and abnormal blood pressure
